Clinical coding abstraction

Abstraction is the process of reviewing the medical records, analyzing the information to determine what is the clinical condition(s) patient had, what caused it and how it was treated. The information needs to be picked from all the sources of data provided like interpreted radiology reports, lab interpretation, follow up status etc.

Process includes 2 phases :

• Determining the appropriate clinical condition / procedures.
• Updating the data into the assigned EMR / clinical software.
